Short Term Assessment and Reablement Service (STARS)

STARS is a short term intensive reablement programme to help you live at home following a stay in hospital or if there has been a change in your care and support needs following an illness. We also offer a rapid response service which offers hands on care and practical support, required by service users and carers at a time when they are unwell to prevent them from going into hospital unnecessarily.  

The STARS service is for anyone over the age of 18 who lives in the Rochdale Borough and is a short-term service lasting for up to six weeks. We will review your progress within the first two weeks of your time with STARS to see if you will need ongoing care and support.

The STARS service is accessible via a referral from the NHS Rapid Response Team, Trusted Assessors within hospitals, and Adult Care Services.

The STARS service is free during the assessment period, but once it has been agreed you have ongoing care and support needs (normally at the two week review stage), you'll be referred for a financial assessment to determine any contribution you may have to pay towards the cost of your care and support.
